At Your first initial visit you will have a complete oral examination where the Doctor and Assistant will chart the existing condition of your teeth. 

X-rays will also be taken if needed.  Dr. Levey will then discuss any findings and explain what treatment, if any, is needed.  He also allows ample time to explain his findings and has time to answer all your questions.  Most initial exams take approximately 1 hour.

All required forms are listed on the right side of the page.  You will need Acrobat Reader (a free program) to read and print these forms.  Please be sure to use the Acrobat Reader Print command and not the browser print command to print out these forms.
